Bring to Team Check-in
Official Game Roster
Six TYPED copies of the official Mt Hood Challenge Roster Form are required at team check-in. NO Exceptions. Roster names and birthdays must be typed. Jersey numbers can be handwritten just prior to each game as those may change between games. Jersey numbers are not required for 7v7 teams. All ‘game’ information can be handwritten since those details may change. The U9 and U10 teams can roster up to 15 players and the U11 and U12 teams can roster up to 18 players. The U13-U19 teams can roster up to 22 players, however, no more than 18 players can be listed on the match roster to play in a particular game so you must cross off any players above the 18 player limit that might be listed on the approved and stamped roster prior to the start of each individual match.
Player Cards OR Their Equivalent
If you are unable to print player cards for some reason, please bring one copy of an official roster generated by your club’s registration system. This does not need to be signed by anyone and it will replace the physical player cards. OR,
You can Type and Print your six copies of the Official Tournament Roster. Bring these to your club registrar and they will confirm all players and adults are properly registered with your state association and insured. They will then need to complete AND sign one copy of our Certified Roster Form. These two items stapled together will replace the physical player card. OR,
Just bring your player cards. We accept any player card from last season or this season. Cards do not need to be laminated, have pictures, or be signed. The birth dates on the cards must match the birth dates on the roster form. We can accept player cards from any of the national governing bodies as long as they are not expired.
We expect every team to follow the policies and procedures established by their club and by whichever national association has issued their player cards. Please check with the appropriate leaders as to which player cards can be used in a tournament and if they allow your team to travel/play with players on the same team that have cards issued by more than one national association.
